Application for Reservation of Columbarium Niche

I hereby apply to reserve one niche in the All Saints Lutheran Church Columbarium. I understand that this application is subject to approval by the Columbarium Committee. I also understand that if this application is accepted, the purchase and use of the niche will be subject to the Columbarium Policies and Regulations. I have been furnished with and have read the Policies and Regulations and understand the Policies and Regulations may be changed at any time by the Congregational Council without notice.
